Why is this important? This is a new therapy that is grounded in high quality scientific evidence. This “Real World” group of patients from five UK hospital sites mirrors the durable restorative clinical outcomes seen in more tightly controlled 5 year follow up studies. It provides supporting evidence for the upcoming NICE review. If successful, this therapy will be rolled out across the NHS. This article builds on the name change of Persistent Spinal Pain Syndrome, where the type 2 variant was formerly known as Failed Back Surgery Syndrome. The type 1 is the same array of conditions but without prior surgery. Why is this article important? PSPS is a collective term for a full range of secondary pain conditions of spinal origin. The traditional anatomical model of understanding spinal pain conditions is not sufficient and often leads to inappropriate treatment such as spinal surgery. We present the PAMC model that is based upon putting the mechanism of pain foremost. The categories generated should improve communication between stakeholders of spinal pain therapy and guide the choice of appropriate therapy.
