Precision targeting is achieved by using anatomical landmarks, live x-ray image intensification or ultrasound guidance. All procedures are carried out within a safe operating clinical environment and usually assisted with light intravenous sedation.

How targeted injections work
Targeted injections apply biologically active medicine (corticosteroid) direct to the site of injury so reducing and eliminating the local inflammation and abnormal nerve fibre activity.

Depot steroid injections have long been targeted to treat local joint chronic inflammation. The pain of sciatica is caused by inflammation around a nerve from a leaking intervertebral disc or compromised nerve root circulation. Local application of depot steroid can treat the underlying cause in some or modulate the dysfunctional sensory system in others. Radiofrequency (microwaves) applied to nerve tissue can change the neurobiology of nerves. This has the effect of selectively reducing pain nerve transmission.

Targeted radiofrequency

Radiofrequency waves are microwaves. However these are targeted onto the sensory nerves. Microwaves cause a change within the nerve and its connections that reduce abnormal pain signals. Sometimes a longer, more permanent effect is achieved with thermal radiofrequency but on some nerves with motor function it is not safe to do so. In which case pulsed radiofrequency is used, which affects the nerves to reduce pain signaling but without the local heat.

 

Procedures

Patients usually have a test/diagnostic procedure before radiofrequency to ascertain and confirm that the nerve targets when anaesthetised do achieve pain relief.

These procedures are carried out in an operating clinical area and made precise with live x-ray image intensifier using radiological landmarks. Confirmation of precise positioning is made with painless nerve stimulation testing.
